APPOINTMENTS AND INSURANCE INFO
Our office offers same day appointments for urgent problems. To reduce your waiting time and aid in office efficiency, our office is organized on an appointment system. If you have a medical problem before or after regular office hours, please call the office number. Our answering service will take your information and your physician will return your call as soon as possible.
WHAT TO BRING TO THE APPOINTMENT
CANCELLATIONS
We ask for at least 24 hours notice when you need to cancel or change your appointment, so that we can offer that time to another patient.
If an emergency causes us to cancel your appointment, we apologize for any inconvenience. We will reschedule your appointment as soon as possible.
INSURANCE, COPAYS AND OTHER PAYMENT INFORMATION
We are providers for Medicare and most major insurance plans. Please verify that we accept your plan before scheduling an appointment. As a courtesy, we will submit your insurance claim for you, however, the basic responsibility for payment is yours and anything not covered by your insurance plan will be your obligation.
Your insurance company requires that we collect your co-payment or deductible at the time you check in for your appointment. For your convenience, we accept credit/debit cards, checks and cash. For cash patients, payment in full is required at the time of service.
